Step1

Patient Form Submission

If you are interested in receiving acupuncture at The Pacific Wellness Institute, please submit a patient form online via a secure internet connection.  Alternately, you may fax or mail the patient form.  It is important that you list your email address accurately and state reachable phone numbers so that we can contact you promptly.

Once we receive your form, it will be reviewed by our acupuncturists.  If we consider acupuncture to be suitable for your case, we will contact you to schedule a consultation appointment.  

We respect your health information and it is securely protected according to our Privacy Policy.

 

Step 2

Consultation appointment

In order to make your consultation most effective and productive please note the following:

You are asked to read information regarding acupuncture prior to your consultation.  Lists of the website links (or printed documents) will be provided prior to the consultation.  The information will answer most of the questions you may have about our acupuncture treatment.*  Our trained front desk staff is also available to answer any general questions about acupuncture or administrative concerns. 

We would like to allocate the majority of the consultation time for interviewing you about your history, so that we will be able to determine whether or not we can accept your case.  Therefore, it is important that you obtain knowledge about the treatment beforehand by carefully reviewing all the information provided prior to the consultation appointment.  If you need further clarification, please feel free to ask any questions during the consultation.

What happens at the consultation appointment?

During the appointment, your acupuncturist will:

• Interview you about your health history
• Conduct assessments, as needed (i.e. tongue observation, orthopedic testing, palpation of pulse and abdomen, etc.) 
• Determine if you would likely benefit from treatment
• Explain about how treatment will be conducted in general, and present initial treatment plan  
• Answer your questions

You will be asked to sign the Privacy Policy and Informed Consent Forms.

*A consultation appointment is typically about 15-20 minutes. If you do not read the provided information beforehand, all the above items may not be covered and an additional consultation appointment may be required.  Further, a separate appointment may be required for patients who would like detailed diet suggestions or are seeking recommendation on supplements or herbal medicine.
